1. General Release and Waiver:
I waive any and all claims against OCABEDZ Football Academy and its staff, including claims of negligence, for any injury, loss, or damage incurred by my child while participating in any academy activities or events.
2. Health and Medical Clearance:
I confirm that my child is in good health, free from any conditions that could impede their participation, and has been medically cleared by a licensed physician to engage in the physically demanding activities associated with football training.
3. Medical Responsibility:
I accept full responsibility for any medical expenses that may be incurred due to any injury my child may sustain during academy activities. This includes any costs related to emergency medical treatment, hospital visits, and physician consultations.
4. Emergency Medical Authorization:
In the event of an emergency where I cannot be reached, I authorize OCABEDZ Football Academy to seek immediate medical treatment for my child. I agree to cover any related costs and understand the academy will make efforts to contact me as soon as possible.
5. Voluntary Participation and Risk Assumption:
I acknowledge and understand that participation in the academy is entirely voluntary. I assume all risks, both known and unknown, associated with my child’s participation, including but not limited to potential physical injury and property damage.
6. Binding Nature of Waiver:
I understand that this waiver is legally binding and applies not only to myself and my child but also to our heirs, executors, and administrators.
7. Photo and Video Release:
I grant OCABEDZ Football Academy permission to photograph and video my child during academy activities. These materials may be used for promotional purposes, including social media, without compensation.
8. Behavioral Agreement:
I understand that my child must adhere to the academy’s code of conduct. Any inappropriate behavior may result in disciplinary action, including dismissal from the academy without a refund.
9. Liability Insurance:
I acknowledge that OCABEDZ Football Academy carries liability insurance, but this does not absolve me of financial responsibility for injuries my child may sustain.
